Heroic T11 tokens

Hello guys,
Since you've killed every boss already I guess you've already figured out which boss drops which token.
Could you please shed some light on the matter?

Weird thing happend to us today, 3 T11.5 tokens droped of Halfus.
And that's realy confusing to me since he always droped only one.

I would really appreciate if you fill this table:
Format - Boss (week before, this week); Example - Halfus Wyrmbreaker (3 chest, 1 chest)

The drops were just recently hotfixed because you wouldn't have been able to get certain pieces by only killing hard modes (you needed the lower ilvl piece from a normal mode and then the heroic piece from the same boss on heroic mode).

this is the fix:

• Heroic set tokens have been moved to earlier bosses on 25-player Heroic difficulty (chest from Halfus, legs from Maloriak, gloves from Magmaw, helm from Nefarian, shoulder from Cho'gall).
• Tokens for normal versions of the helm and shoulder set pieces (item level 359) now drop from Nefarian and Cho'gall on Heroic difficulty, as they are required to purchase the Heroic helm and shoulder pieces (item level 372). An additional change to this functionality is slated for patch 4.0.6.

Yeah, I've read a blue post on that and it said that nefarian now drops normal pieces as well as heroic.
And there was a list of other bosses who drow pieces like: Magmaw - hands, Maloriak - pants, Halfus - chest.
Is that a complete list? It seems a bit short, even taking in consideration 4Winds and its allaround use tokens.

Our raid from last night seems to indicate that that is correct, only those bosses dropped said tokens. Also, Sinestra doesn't drop any tokens but Al'akir Drops 3 Essence of the Forlorn in 25HC and presumably 1 in 10HC.
